It is important to the team at WARMSPRINGS that you understand that we feel each and every charitable activity is both valid and gracious. We receive hundreds of requests each year and we unfortunately cannot respond in a positive way to all of them. You must complete the entire application knowing that we will only consider requests which have the completed application form.

Please complete the application and send it to us no later than TWO weeks prior to the date of your event. We require this lead time to process your request and supply you with your donation.

Gift Certificates must be redeemed for the services or items listed, as applicable. There is no cash surrender value for certificates and no exchange or extensions after the expire date. Lost or stolen gift certificates cannot be replaced.
